How to Become a Participating Charity

It is simple! All you have to do is complete the form below.

This form serves two important functions for nonprofit organizations.

First, it is the official sign-up form. It tells us how your organization plans to take advantage of the LEAVE A LEGACY^® campaign.
Second, once you complete this form, your organization will appear on our web site. Every participating nonprofit organization will be listed. LEAVE A LEGACY^® will link to your site if you have one.
