- First, it is the official sign-up form. It tells us how your organization or firm plans to take advantage of the LEAVE A LEGACY® campaign.
- Second, once you complete this form, your organization will appear on our web site. Every participating nonprofit organization will be listed. LEAVE A LEGACY® will link to your site if you have one.
